Disease Pharmacotherapy Traineeship
General Program Information
The
traineeship is open to pharmacists from all practice settings.
There will be two traineeship sessions in 2008: June
2-6 and September 15-19. Submission
of an application indicates an ability to attend either session.
An applicant’s preference will be taken into consideration,
but cannot be guaranteed. Four pharmacists will be selected
for each session.
The
traineeship is four and a half days in length, Monday through Friday. There is
a mandatory orientation
meeting on Sunday evening. The traineeship will be conducted
at the Parkinson’s Disease and Movement Disorders Center, Northwestern University Feinberg School of Medicine, Chicago, Illinois.
The Parkinson’s Disease Pharmacotherapy Traineeship is
supported by an educational grant from Teva
Neuroscience.
DEADLINE
FOR RECEIPT OF APPLICATIONS IS MARCH 28, 2008.
Please read this information and the Policies and Guidelines
carefully so you have a clear understanding of the traineeship
goals, structure, and content, and the application procedure.
Trainees are responsible for all expenses related to travel, accommodations, meals, and incidentals. Trainees must make their own travel arrangements to the traineeship site. The ASCP Foundation will coordinate hotel accommodations, but trainees are responsible for payment. Trainees should plan to arrive no later than 3:00 p.m. Sunday
preceding the traineeship for an evening orientation session;
departure
is after 12:00 p.m. on Friday.
PLEASE
NOTE THE FOLLOWING IMPORTANT INFORMATION
- The Parkinson’s Disease Pharmacotherapy Traineeship
is an in-depth program requiring familiarity with pre-traineeship
reading materials and interaction with patients and the interdisciplinary
team at the traineeship sites.
- The primary focus of
the traineeship is Parkinson’s
disease, although other movement disorders and conditions
will be encountered.
- After the traineeship,
participants are expected to prepare two case consults for
which they made recommendations for
treatment or interventions to improve care of patients with
Parkinson’s disease.
Please submit only the material requested;
do not add binders, dividers, printed brochures,
or booklets.
The
application form and required exhibits must be received by the
ASCP Foundation no later than March 28.

If
you mail your application and would like to receive confirmation
that it has been received, please enclose with your application
a self-addressed post card with postage affixed. Applications
for the traineeship will be evaluated, and applicants
will be notified of the outcome on or about April 7.
