Traineeship
General
Program Information
The
traineeship is open to pharmacists from all practice settings.
The traineeship will be held October 15-19. 2007.
Four
pharmacists
will be selected for participation.
The
traineeship is five days in length, Monday through Friday,
with
frequent evening sessions. There is a mandatory orientation
meeting on Sunday evening. The traineeship will be conducted
at the Mood and Memory Clinic of Michigan and Botsford General
Hospital in Farmington Hills, Michigan, and Oakwood Nursing
Homes in Dearborn and Trent, Michigan. The Alzheimer’s/Dementia
Traineeship is supported by an educational grant from Forest Laboratories.
DEADLINE
FOR RECEIPT OF APPLICATIONS IS AUGUST 10, 2007 .
Please
read this information and the Policies and Guidelines carefully
so you have a clear understanding of the traineeship goals,
structure, and content, and the application procedure.
The
trainees are responsible for all expenses related to travel, accommodations,
meals, and incidentals. Trainees must make their
own travel arrangements to the traineeship site. The ASCP Foundation will coordinate hotel accommodations, but trainees are responsible for payment. Trainees
should plan to arrive no later than 3:00 p.m. Sunday preceding
the traineeship
for an evening orientation session; departure is after 5:00
p.m. Friday.
PLEASE
NOTE THE FOLLOWING IMPORTANT INFORMATION
- The
Alzheimer’s/Dementia Traineeship is an in-depth
program requiring familiarity with pre-traineeship reading
materials and interaction with patients, family members,
and the interdisciplinary team at the traineeship sites.
- Applicants
must be willing to interact directly with patients suffering
from various stages of Alzheimer's disease, their caregivers,
and their families in a variety of settings.
- The
primary focus of the traineeship is Alzheimer's disease
and related dementias, although comorbid conditions will
be encountered.
- After
the traineeship, participants are expected to become involved
with their local Alzheimer's Association and complete two
case consults for which they made recommmendations for treatment
or interventions to improve care of patients with Alzheimer's
disease.
Please submit only the materials requested; do not add
binders, dividers, printed brochures, or booklets.
The application form and required exhibits must be received
by the ASCP Foundation no later than August 10.

If

card, affix postage, and submit with your application. Applications
for the traineeship will be evaluated, and applicants will
be
notified of the outcome on or about August 24
